Backache is the pain felt over the back which is divided into neck pain, middle back pain, or coccydynia based on the area affected. The lumbar area is the most commonly affected. It is a clinical condition that hinders the working capacity of middle-aged men or women.


Cause

There are many causes of back pain. But in most cases, the cause of backache is a non-specific origin, which may be due to back strain or sprain. Some of the causes listed are herniated disc or stenosis, degenerative changes, traumatic attack on vertebrae, Metastatic cancer, osteomyelitis, and epidural abscess. In some cases, the acute or sub-acute back ache may end up in chronic pain.

Poor nutrition, fatigue, sedentary lifestyle, obesity, heavy weightlifting, lack of exercise, smoking, psychological factors such as job stress, family issues, etc, poor posture, pregnancy, and after delivery are some of the risk factors for back pain.

History of cancer, fever, unexpected weight loss, immunosuppression, urinary infection, retention or incontinence, fecal incontinence, bilateral weakness of legs, etc is causes or associated symptoms which signal serious underlying pathology. These need proper diagnosis and emergency management if needed.

Pathophysiology Degenerative or traumatic changes formed in discs are one of the most common causes. Inflammation of any diseases related to the Gall bladder, pancreas, aorta, and kidneys may also cause referred pain in the back.

Inflammatory changes, tumor of vertebrae, neural tissues, and adjacent structures also manifests back ache. Some of the inflammatory changes may be autoimmune as in Psoriatic arthritis, RA, SLE, and Ankylosing spondylosis. Signs & symptoms Characteristic features of back ache depend on the severity and origin of the cause. Back pain may be acute, sub-acute, or chronic depending on the duration of onset. Some of the features of back ache may be characteristic dull, shooting or piercing ache in the adjacent muscles and nerves, burning sensation, discomfort, numbness, weakness, referred or radiating pain to the bones, joints, and extremities. Classification Bach pain is basically classified on the duration namely; Acute (< 6 weeks), Subacute (6-12 weeks), and Chronic (> 12 weeks) Ayurvedic Management of Back Ache

In Ayurveda Backache generally come under Vata disorders. Ayurveda internal and external treatment successfully manages Backache due to inflammatory changes and traumatic changes in the spine. Another backache with an underlying cause of disease may be taken into consideration of its severity and chronicity.

Ayurveda internal medicines along with systematic Panchakarma treatment and dietary regulations could reduce the oedematous or inflammatory conditions of the affected person. Special massage will be administered in order to pacify the muscular pain and radiating or numbness of adjacent extremities.
